WHAT TO BRING TO CAMP

Health Forms

We require that all campers have current and updated health forms on file to attend camp. All forms must be completed and turned in either by emailing a copy to the Camp Director lily.eighmie@polaricenc.com or they can be turned in on the first day that your camper attends.

Please note these forms are separate from the registration process through DaySmart. If you have any questions or need to check to see if your forms are current, please contact Allison Prince.
